Diabetes Management

At Charted Care, we recognize that effective diabetes management requires a comprehensive approach that includes education, tools, and support. Our goal is to empower individuals living with diabetes to take charge of their health through a variety of specialized services tailored to meet their unique needs.

Diabetes Education

Our diabetes education program is designed to provide individuals with the knowledge and skills necessary to manage their condition effectively.

Our certified diabetes educators deliver evidence-based information on topics such as blood glucose monitoring, medication management, lifestyle modifications, and the prevention of complications. Through interactive sessions, patients learn how to make informed
decisions about their health.

Understanding how to use diabetes devices is crucial for effective self-management. We offer comprehensive training on a variety of devices, including:

Insulin Pens: Instruction on proper usage and dosage administration.
Glucose Monitoring Sensors: Training on continuous glucose monitors (CGMs) and traditional
glucometers, including interpreting results.

We are contracted with multiple insulin pump companies and provide specialized training on the use of insulin pumps. Our trainers guide patients through the setup, programming, and troubleshooting of their pumps, ensuring that users can seamlessly integrate insulin pumps into their daily routines.

Continuous support is available to address any questions or concerns post-training.

Nutrition plays a vital role in diabetes management. Our registered dietitians offer Medical Nutrition Therapy (MNT) to help patients develop personalized meal plans that align with their health goals.

This service includes education on carbohydrate counting, understanding food labels, and incorporating nutritious foods into daily meals to maintain optimal blood sugar levels.

Managing diabetes often involves addressing coexisting chronic conditions. Our integrated chronic disease management program focuses on coordinating care for patients with diabetes and other related conditions (e.g., hypertension, obesity, and dyslipidemia), promoting overall health and well-being through individualized care plans.

We offer in-servicing and presentations for healthcare professionals to enhance their understanding of diabetes management.

Our expert educators can provide tailored training sessions, workshops, and lectures that cover the latest advancements in diabetes care, device technology, nutrition, and patient education strategies.
